Thank you for choosing The Laser and Surgery Center for your eye surgery. We strive to make your experience a pleasant and efficient one. With that in mind, we like to inform our patients of their financial obligations prior to surgery, so there are no surprises and/or misunderstandings.
Notice of Payment Estimate Requests
Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the Laser & Surgery Center to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.

Insurance & Billing Information
Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as the facility.

The Laser & Surgery Center utilizes the pathology services of Bay Pathology Associates if your care requires pathology services. They can be contacted direct at (850)-769-7963 or 760 Airport Drive Panama City, FL 32405.

Payment & Financing Options
We collect any co-payments, deductibles, and co-insurance amounts on the day of surgery. As a reminder, you are responsible for any balance not covered by your insurance company. If you do not have insurance, you are responsible for all fees incurred with the surgeon, facility and anesthesia. You will need to arrange payment with each entity separately. We accept cash checks, and credit cards. Affordable financing is available through CareCredit. CareCredit is the credit card exclusively designed for healthcare services.
